If I run a server with custom maps, do the clients need it as well, or are all maps purely server side?
If so, do clients automatically download it - or does it work like Tribes 2 - same terrain different buildings?

yes. clients do not need to have the maps

That's very cool.
There was a server up called Buff and Lost maps - quite amazing stuff, they even had automatic tanks and planes flying around defending the base.
When the maps are custom - is the terrain the same as well - or is the game client able to render everything the server map specifies?

the terrains are not custom, they are the terrains that came with the game*.
your terminology is not the norm in the novalogic gaming community quiet, yours is probably more correct.
given that, i hope that i understood what you were asking & answered it correctly...

*note:
you can make custom terrains, but they must then be installed by each client. please.... you don't want to go there !

Oh yes, that makes perfect sense.
The game has all the objects in files, so basically the custom maps merely says put this object here or there and do this or that with it.
Since the terrain is the same as the other maps, no client download necessary.
I'll try to make more sense in the future. Sorry about that. :)
